Welcome
Senior Outreach Services
Senior Outreach Services is our newest program. It reaches out to seniors who wish to keep their independence but need that extra assistance in order to do so. A trained member of our program visits with seniors and their families in the comfort of the senior’s home to talk about their needs and to develop a Care Plan.
We look at all areas of the senior’s life: physical, emotional, social, financial, spiritual, family and their cognitive ability. The trained member of our program will coordinate the use of the community resources and follow-up with the senior and their family to ensure that their needs are being met. We also provide emotional support and in-home counseling services to alleviate symptoms of depression, anxiety, and other issues that decrease a senior’s ability to enjoy their lives. We can also help navigate the confusion governmental programs and the health care systems to ensure those seniors and the families are enjoying the highest quality of life.
Our Mission
It is our mission to serve Homebound Seniors in the Northwestern Comal County and Comal ISD areas, by providing resources and services that restore their independence and quality of life.
Our Clients
